Disclosures
Please send all relevant original documentation to Enterprise Care via recorded delivery; also make sure that a self addressed, prepaid recorded envelope is enclosed for the safe return of documents. Payment must be sent with completed form/forms.
If you are an agency/organisation you will be required to sign a declaration to verify that you have seen the person and identification of the person who requires the disclosure. It is also the duty of the agency/organisation to ensure that the disclosure forms are checked and completed following the information in the Disclosure Guidance Booklet before sending them to us. Failure to do so will result in an additional administration charge of £3.99.
If you are an agency/organisation, POVA first checks are available on request. These will be applied for once the applications have been sent to the CRB, (following CRB guidelines) and do not need any further action on your part. The results of these checks will be disclosed to you initially by telephone or email; however a hard copy can be posted if required. For more information about this check please refer to the CRB website.
Single applicants who do not have anyone to verify their ID will be required to bring the application to us in person, along with identification, so that we are able to verify this ourselves.
